LIBRARY REGISTRATION

Library Rules & Copyright Act (1968)

I declare that all copies requested by me under this agreement are for the purpose of research, study or business at either Central Gippsland Health or Bairnsdale Regional Health Service and I will not use them for any other purpose, and I have not previously been supplied with a reproduction of the same material by an authorised officer of The  Library. In the event that my request is for more than one article from the same issue of a periodical, I further declare that each article is for the same research or course of study. When more than a reasonable portion of a work is requested I declare that, after reasonable investigation, I am satisfied that a reproduction (not being a second-hand reproduction) of the work cannot be obtained within a reasonable time at an ordinary commercial price.

I agree to abide by the rules of the CGH/BRHS Library and accept the following conditions:

  1. The Library will maintain a record of this information, such as my name, organisation, address, phone number and email which will be used to manage my loan transactions, respond to my enquiries, and support me in my role.

  2. I accept responsibility for the items I borrow from the library and shall not lend items to other persons or organisations. An email alert will advise the due date for item/s; should I damage or fail to return items by the due date that I may be charged a replacement/processing cost. 

  3. The Library shall issue me with Login and Password credentials which will I not will share or distributed to others. 

  4. Should I breach a Library rule, access and borrowing rights may be withdrawn and my employer/course provider notified. 

  5. The Library is permitted for security/statistical purposes, to maintain a record of my access of The Library. 

  6. I am familiar and understand the applicable organisational ICT access/use process; being  BRHS ICT Network Access policy &/or  CGH Use of Information and Communication Technologies procedure.
